What does differentiate is who is behind it: names, faces, years in practice and a specific way of treating people. It is the first thing someone checks before letting a stranger work on their mouth, and it is exactly what most dental sites bury on a subpage.
Most visits to a dental site come from a phone, many of them outside opening hours and quite a few from someone in discomfort. An eight-field form loses that person.
